The Document of Gift and Informed Consent is used by a living donor to authorize "anatomical donation" of his or her body.  There are a lot of required disclosures on the form in order to make sure the Donor is fully informed.  The disclosures were obviously not designed with cryonics in mind.  The large text in section 9 is required to be that size by Oregon Law.

Ignore the following document if you are signing up for membership.  In certain rare situations, as part of a compromise with family members, only the brain can be sent to Oregon Cryonics.  This document of gift form is tailored for those situations.
