If the donor is still living, then he or she should fill out and return a Document of Gift and Informed Consent.

If the donor is deceased, then an Authorizing Person should fill out and return a Document of Authorization.

Placing in Chart

These documents are stored in the paper chart.  If deceased or pending, then also scan into the computerized chart.
